In accordance with Internal Revenue Code Sections 6055 and 6056 and applicable IRS guidance, Row Partners has prepared IRS Form 1095-C for the 2025 calendar year to report information about health coverage. This notice also applies to IRS Form 1095-B for individuals enrolled in our self-insured health plan.

You may request a copy of your Form 1095-C/1095-B, and it will be timely furnished within 30 days of the request. If you consent to electronic delivery, your form may be provided electronically.
