What is a health application?

A health application is an important part of the benefits process. It provides the information needed to:

• Confirm eligibility and enrollment for employees and eligible dependents.
• Meet applicable carrier and regulatory requirements.
• Support accurate underwriting of the group health plan.
• Help determine available plan options and potential future rates.

Each eligible employee completes their own application directly and may also choose to waive coverage by completing the Row Partners Waiver of Coverage Form sent to them.

Your employees’ information is protected

At this step, you do not need to provide any employee health information. We only need each eligible employee’s email address so the appropriate forms can be sent directly to them.

Health applications and coverage waivers are completed through a secure process. Any protected health information (PHI) submitted as part of the application process is treated as confidential and handled through secure safeguards designed for protected information.
