Get Started with Health Insurance with a basic census

A census provides the demographic information needed for carriers to evaluate your group and develop available health plan options and rates.

The census should include the following information for all eligible participants, including employees and eligible dependents (spouse(s) & Child(ren)):

  • First Name
  • Last Name
  • Date of Birth
  • Gender
  • ZIP Code

This information helps underwriters collect the information required as part of the underwriting and quoting process. It provides the information needed to:

  • Confirm eligibility and enrollment for employees and eligible dependents.
  • Meet applicable carrier and regulatory requirements.
  • Support accurate underwriting of the group health plan.
  • Help determine available plan options and potential future rates.

What should I include?

Please include each eligible employee and any eligible dependents who should be represented in the group census.

At this stage, you do not need to provide any medical history, health conditions, or other health information. The census only collects the basic demographic information needed to begin evaluating coverage options for your group.
